REQUIRED ID:

We need to see ID and a document with your name and current address on it, such as a recent bill. If you are unable to provide ID/current address, please speak with our registration volunteers.

 Here are some examples of ID: 

• Employee ID

• Insurance Card

• Driver license or passport

• Other government ID

• Birth Certificate

• Social Security card

• Vaccination record

• School ID or letter from the school
